After that disaster, I implemented a new verification protocol for all incoming plastic materials. It took effect in January 2020. Basically, I split our specs into two levels. It's knowing that the material will run on your line the same way every time.

And if a vendor can't give you detailed data on thermal stability, provide a pilot run sample, or guarantee a specific plastic formulation (like a rigid PVC grade or a specific PET type), that's a red flag in my book.
